Appointments
If you would like to make an appointment, please call our office at 818-360-3636. You can also fill out the Appointment Request form in the Contact section.
Insurance
We accept a wide range of insurances. Please call the office to see if we can accommodate your plan.
Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
Financial
To best serve you, we have established several financial arrangements. Payment in full at the begining of the treatment will be discounted 7-10 percent. Payment plans are available over the course of your treatment with a down payment and electronic fund transfer (No late fees or statements!). VISA and MASTERCARD accepted and CareCredit is available.
